First Visit Check List

• Street parking is usually available out front, back, and around the corner. You can also park in the Courthouse Plaza surface lot closest to the AMC Courthouse Movie Theater.

• Our office is located on the 3rd floor (suite 320) of “JBG Smith- Courthouse Plaza II” Building. Also known as, the building with FedEx and Courthaus Social on the ground level.

• If you didn’t return first visit documents through your PT Everywhere account, arrive 15-minutes prior to appointment for completion

• If you have a script/referral/physician order for physical therapy, bring it with you or upload it to the document center of your PT Everywhere account prior to visit

  • this can include post-operative protocols, image and/or other medical testing reports

• Wear comfortable clothing that is easy to move in

  • Short Sleeves or Sleeveless Shirts for upper quarter evaluations

  • Shorts or pants that can be pulled up over your knees for lower quarter evaluations

• Payment is due at the time of service.
